- Must buy something at Nike in-store or online lately.
- Receive a valid purchase receipt from that shopping with a survey invitation offer printed on it.
- Must complete the survey within the next 7 days of receiving the invite as the survey entry code expires seven days from the date of purchase.
- Must not be an employee of Nike, Market Force, and its associated organization or any of their immediate family members or any person involved in the survey.

- Visit the Survey site on your Nike receipt. Or, go to www.mynikevisit-na.com
- Click on a language among the provided option to continue the survey.
- Type the Enter code from your survey invitation receipt. (take a glance at the sample receipt image above the entry field to know where to locate the information on your paper receipt)
- Click on the “NEXT” button.
- Respond to the survey questions to the best of your knowledge.
- Write the validation code in the provided space on your receipt, when it appears on your computer screen at the end of the survey.
A Few Important Notes on Nike Online Survey
- The offer is limit to 2 completed surveys per household in a 7 days period
- Offer valid at any Nike retail location; void where prohibited
- To redeem the offer you must present the original survey invitation receipt along with the code received at the end of the survey.
- The Validation Code expires after a certain period of time so you should redeem the discount as soon as possible after taking the survey online.
